Your insurance will be billed directly for covered services. Please provide us with your current insurance card at each visit.
We are happy to file the claim for you, but the balance is your responsibility if your insurance company or plan does not pay after 30 days. Any amount not covered by insurance is due immediately after you recieve the first invoice from us.
We accept Medicare patients. As a courtesy, we will also bill your secondary insurance directly. You will be asked to sign an Advanced Beneficiary Notice for services not covered under Medicare. You shall be accountable for covering these services.
We also accept most major insurances including, (but not only) Premera, Regence Blue Sheild, Blue Cross/Blue Sheild, Aetna, Cigna, United Health Care, AARP, SoundPath.
